Instructions

• The Das-Churchville ORB Checklist© is an observational tool, not a formal diagnostic instrument, and has not been validated with established norms. If a child demonstrates a significant number of the listed characteristics, a professional assessment is recommended.
• For each item, indicate the observed frequency or intensity of the behavior using the following scale: 0 (Not Observed), 1 (Rarely), 2 (Sometimes), 3 (Frequently), or 4 (Consistently).
• Scores for each subsection, section, and the final overall total are automatically calculated.
• NOTE: Always consider the student's age and developmental stage when interpreting observations.
