The HAGT PRISM© Questionnaire (Profile of Reasoning, Interests, Supports, & Motivation) is a free, interactive tool that generates a personalized learning profile for your child, revealing their interests, thinking patterns, motivations, and ideal learning environment.
Developed by the HAGT Collaborative, it is designed for students from Grade 2 through Grade 8 and explores four dimensions of how your child engages with the world, turning their responses into a rich profile you can use to support them at home and advocate for them at school. Every child thinks and learns differently. PRISM© helps you see exactly how.
What the PRISM© Snapshot Includes

Create Your Free Account
Getting started takes fewer than two minutes. Sign up by entering your email address; once you confirm it, you will be directed straight to the questionnaire. Your account is where your child's profile will be saved, so you can return to it anytime.

Your Child Takes the Questionnaire
The questionnaire is scenario-based and interactive, with age-appropriate versions for younger learners (Grade 2 to Grade 4) and older students (Grade 5 to Grade 8). It typically takes 20 to 40 minutes for your child to complete and feels more like an exploration than a test.

Their PRISM© Snapshot Is Generated
Once complete, a personalized snapshot is created and saved to your account. Log back in anytime to view it. Use the free snapshot to start conversations at home and at school or go deeper with an optional detailed report and a one-on-one consultation with our HAGT specialist.

Once your child completes the HAGT PRISM© Questionnaire, they receive a personalized free PRISM© Snapshot. Everything in the PRISM© Snapshot is visual, accessible, and designed to be used as a reference. It is saved to your account, accessible anytime, and covers:

Once you are on the PRISM© page, you will need to create a free account to get started. You will find the option to create an account under the sign in button. It takes less than two minutes to set up.

Your child's PRISM© Snapshot is saved to your account automatically once the questionnaire is complete. You will be directed to the dashboard with the results of your child’s PRISM© Snapshot as soon as the questionnaire is complete. Log back in at any time at to view it. There is no download option, so make sure you hold onto your login details.

The PRISM© Snapshot is an overview covering your child's dominant areas of curiosity, how they approach problems and process information, what drives and motivates them, and the learning environment where they do their best work. It is designed to be a starting point for conversation, with your child, their teacher, or an HAGT specialist.

Yes, and we encourage it. It can be a useful reference point for parent-teacher conversations, IEP meetings, or any discussion about how to better support your child in the classroom.

The PRISM© Profile is a paid option for families who want to go beyond the free Snapshot. It pairs a comprehensive written report with a 50-minute one-on-one consultation with an HAGT Collaborative specialist.

The report provides a comprehensive written interpretation of your child's results, context for what each dimension means, and personalized recommendations for home and school. It goes beyond the Snapshot to give you something you can actively use in conversations with teachers or specialists.

You will meet with an HAGT Collaborative specialist to walk through your child's results and report together, ask questions, and discuss next steps. Sessions are conducted online and last 50 minutes.

Once your child's PRISM© Snapshot is ready, simply scroll down within your account to find the option to book a consultation and upgrade to the full PRISM© Profile.
